The easiest way to remember HMRs is to refer at visits you already have booked.
75+ health assessments
If the assessment turns up several medicines, a fall, or confusion about doses, refer for an HMR at the same visit.
Chronic condition management plans
When you prepare or review a GPCCMP (items 965 and 967) and goals aren't being met, add an HMR to the plan.
After a hospital stay
Medicines often change at discharge. An HMR checks what is actually being taken at home against the discharge summary.

Questions GPs ask
How soon will my patient be seen, and when do I get the report?
We phone your patient within 2 business days of your referral and usually visit within 2 weeks. The report usually reaches you within 7 days of the visit. If we find something urgent, we phone you the same day.
Is there any cost to my patient?
No. HMRs are funded by the Australian Government, so there's nothing for your patient to pay for the review.
What do I claim, and when?
Once you have the report, discuss it with your patient, agree a written medication management plan, and claim MBS item 900. It can be claimed once in 12 months, unless the patient's condition or medicines change significantly. From 1 July 2027, a GPCCMP will also be required to access item 900.
What if I disagree with a recommendation?
You stay the prescriber. The report is advisory: you decide what, if anything, to act on, and the plan column lets you record "no action required" against any finding. We're always happy to talk a finding through.
Who contacts the patient?
We do. Once your referral arrives, we phone the patient (or their carer) to explain the review and book a time that suits them. You don't need to do anything else.
Does the referral have to be on your form?
No. Any referral with the patient's details, the reason for referral and a current medicine list works. Our online form and software templates just make it quicker.
What about patients in residential aged care?
They need a Residential Medication Management Review (RMMR) instead. See RMMR referrals.

We're happy to come to you, free, anywhere across Melbourne and the Gold Coast: a short visit to introduce ourselves, a lunch & learn on medication safety for your GPs and nurses, or a quick call. We can also help your practice manager set up the HMR recall kit.
