Understanding the service

What is a Home Medicines Review?

A simple, no-cost service that brings expert medication advice to your living room.

A Home Medicines Review (HMR) is a free, in-home consultation with an accredited pharmacist. We sit down with you (and your carer or family, if you like) to go through every medicine you take — prescription, over-the-counter, and complementary — and check that everything is working well together.

After the visit, we prepare a detailed report for your GP with practical recommendations, which may include simplifying your medication routine, addressing side effects, improving adherence, and resolving any interactions or duplications.

An HMR is especially valuable for people taking multiple medicines, those who have recently left hospital, people managing several health conditions, and older adults or anyone supported by a carer.

Who can benefit?

  • People taking five or more regular medicines
  • Anyone with a recent hospital admission or change in medicines
  • People with multiple chronic health conditions
  • Older adults, carers, and those finding medicines confusing

Eligibility

To receive an HMR, you'll need a referral from your regular GP. If you think you may benefit, simply mention it at your next appointment — or get in touch with us and we can help coordinate the referral with your GP's clinic.

Are you a GP? See our referral page for the pathway and a printable referral form, or add our template to Best Practice, MedicalDirector or Zedmed.

In your language

How It Works

Getting started is easy — and there's no cost to you.

1

Your GP refers you

Your doctor identifies that you may benefit from an HMR and provides a referral, with your consent.

2

We visit you at home

At a time that suits you, the pharmacist visits to discuss your medicines, health goals, and any concerns.

3

Report & recommendations

A detailed report with practical recommendations is sent to your GP, who follows up with you on next steps.

What to expect on the day

  1. We arrive and say hello
A friendly, unhurried start — a family member or carer is very welcome to join us at the kitchen table.
  2. We go through every medicine together
Prescriptions, over-the-counter medicines, vitamins and supplements — what each one is for, how it's taken, and how it's working for you.
  3. We look at routines and storage
Where medicines are kept, expiry dates, doses that are easy to miss, and simple ways to make the routine easier.
  4. Your questions, answered plainly
Side effects, timing, food interactions — no question is too small, and nothing changes without your GP's say-so.
  5. Your GP receives the report
Within days, your GP has a clear, practical report and follows up with you on any recommended changes.

What's involved

What a review covers

Every HMR is different, because every medicine list is different. These are the things we always look at.

Everything you take

Prescription medicines, over-the-counter products, vitamins, and complementary medicines — checked together, because interactions don't respect the boundary between them.

Whether it still suits you

Medicines that made sense years ago may not now. Where something looks unnecessary or risky, we set out the options and the reasoning for your GP to consider.

How the routine actually works

Doses that are hard to remember, packaging that's hard to open, timing that clashes with meals or sleep. A plan you can't follow isn't a plan.

A written report to your GP — findings and suggestions, sent to the doctor who referred you, so any changes are made by them.
A plain-language summary for you — what each medicine is for, in writing, to keep and to share with family or carers.
No cost to you — HMRs are funded through Medicare when your GP refers you. There is no out-of-pocket fee.

Any change to your medicines is always a decision for you and your GP — we advise, your doctor prescribes.

60-second check

Could you benefit from a review?

Tick anything that applies to you (or the person you care for).

This is a guide only, not medical advice — your GP makes the final call on whether an HMR is right for you.

Good to know

Frequently Asked Questions

Answers to the questions we hear most often.

Does a Home Medicines Review cost anything?

No. HMRs are funded by the Australian Government under the PBS, so the service is bulk-billed — there is no cost to you when you have a valid referral from your GP.

How do I get a referral?

Your GP arranges the referral. Simply mention at your next appointment that you'd like a Home Medicines Review, or get in touch with us and we can help coordinate it with your GP's clinic.

What should I have ready for the visit?

Have all your medicines on hand — including prescription, over-the-counter, vitamins, and supplements — along with any recent letters or lists from your doctor or hospital. A family member or carer is welcome to join.

How long does the visit take?

Most visits take around 45–60 minutes. We never rush — the goal is to understand your full picture and answer all of your questions.

Is my information kept private?

Yes. Your information is handled confidentially and used only to prepare your review and report for your GP, in line with Australian privacy law.

What happens after the review?

We send a detailed report with practical recommendations to your GP, who discusses any suggested changes with you and updates your management plan as needed.
