Before the visit, gather these in one spot

Every medicine you take — prescriptions, over-the-counter, vitamins, and herbal products.
The easy-to-miss ones — patches, creams, drops, sprays, inhalers, and injections.
Anything in the fridge — insulin, some eye drops and injections live there.
Medicines you've stopped or only take now and then — they matter too.
Your Webster-pak or dosette box, if you use one.
Your Medicare card.
A list of your questions — anything that worries you about your medicines is worth raising.

Good to know

The visit takes about an hour, in your own home, at no cost to you — it's covered by Medicare through your GP's referral.

A family member or carer is very welcome to join us. Nothing changes without your doctor: after the visit we send your GP a report, and any changes are decided together with you at your next appointment.
